The slab has sunk enough to cause a gap between the slab and the garage door. They decided to call EFS for a free inspection. 

Design Specialist Stephen Caron inspected the garage and confirmed that there was some cracking in the mouth of the garage and it needed to be lifted as well. Stephen recommended installing PolyLevel to lift the slab, and then using Nexus Pro and Concrete to repair the cracks on the slab.
